Housing Stabilization Services

If you are disabled, have mental illness, substance use disorder, you a senior citizen, and have Minnesota Medical Assistance (MA) or related assistance, Avid Community Care can help you find and sustain housing.

Living an Independent Life

People with disabilities often need support to live successfully in the community. However, that support is often unavailable. Affordable housing is not always enough; challenges such as mental illness and developmental disabilities can make it difficult for someone to find housing, budget, interact with landlords and neighbors, and understand the rules of a lease. With the right supports, provided by a professional with knowledge and experience in housing, more people can be successful.

Assisting Clients with New Benefits

At Avid Community Care, we provide Housing Stabilization Services (HSS) to our new and existing clients. HSS is a new Minnesota MA benefit that helps people with disabilities, mental illness and substance use disorder, and seniors find and keep housing.

What are Housing Stabilization Services?

Various challenges and barriers can make it hard to find housing, budget, interact with landlords and neighbors, and understand the rules of a lease. Housing Stabilization Services is a Medical Assistance benefit designed to help people with disabilities and seniors find and keep housing.

Housing Consultation

‍These are services that assist a person in developing a housing focused person-centered plan, assist the person to access needed state plan services that support housing stability and provide referrals or information about to other needed services. People who do not have waiver or targeted case management or care coordinator, but need Housing Stabilization Services, can complete the person-centered planning needed through an enrolled housing consultant. Consultants help develop a Housing Focused Person-Centered Plan (DHS-7307) and support the person to select their housing transition or sustaining provider.

Housing Transition

‍These are services that assist a person to plan for, find and move to a home in the community.

Housing Sustaining

‍These are services that supports a person to maintain living in their home in the community.
