Home and Community-Based Services (245D)

Avid Community Care provides a variety of services for individuals with various needs. The Home Health Care services we provide remain the foundation to all of our services resulting long term benefits for our clients. Whether we are providing Personal Care attendance (PCA)  or Independent Living Skills (ILS), Night Supervision, (NS) Personal Support ), Employment Services and Supports (CTSS), or Respite services our staff always considers the wellbeing of the individuals and families we serve.

Recuperative Care

At Avid Community Care, Recuperative Care, we understand the challenges faced by individuals who are homeless and suffering from acute medical conditions. It is with these challenges in mind that we designed our comprehensive Recuperative Care Program. Our primary objective is to provide ongoing health and treatment services to patients who no longer require hospitalization but still need specialized care and support in a conducive recovery environment.

Our program provides more than just medical care. We understand that healing is a multifaceted process that involves more than treating the symptoms of an illness or injury. We offer regular health assessments, case management, assistance with transitioning to safe housing, and transportation to and from medical appointments.

For many years, individuals experiencing homelessness were often discharged from hospitals back onto the streets or into shelters that were ill-equipped to manage post-hospital care. This often led to a vicious cycle of repeated hospital visits and further deteriorated health conditions. At Avid Community Care, we strive to break this cycle and provide the comprehensive care that these individuals deserve.

In-Home Support

Avid Community Care is committed to providing well-rounded surveillance for the health and well-being of our clients. To ensure that you stay on top of your health at all times while staying at home, we provide extensive support that fits your preferences and situation. We also see to it that your needs are well attended 24 hours a day and 7 days a week by our caregivers.

Our in-home support services include the following: In-Home Family Support Independent living skills training Individualized Home Supports Semi-independent living skills Supported Living Services for Adults And more.

Independent

Independent Living Skills (ILS) Services

Provided to address identified skill development needs of a person in the areas of communication, community living and mobility, interpersonal skills, reduction/elimination of maladaptive behavior, self-care, sensory/motor development involved in acquiring functional skills. Services are provided in the home of the client at the time of their choosing or agreed upon by the worker and client.

Employment

Supported Employment Services (SES)

Individuals served in the Supported Employment Services (SES) division engage in a variety of settings where persons without disabilities are employed. Services provided include an individualized work-related assessment, on-the-job training in both work and related skills, ongoing supervision and monitoring, and other supports to assure job acquisition and retention. The outcome is to help people maintain paid employment in community businesses/settings. Employment support services occur in integrated community setting.

Night

Night Supervision

Overnight assistance and monitoring provided by an awake staff in the person’s own home. Night supervision services include an awake staff in the person’s own home when the person needs assistance and supervision in at least one of the following areas:

Carrying out the person’s positive support programming and transition plans
Reinforcing skill development supports (e.g., individualized home supports)
Assisting with activities of daily living (ADLs)
Assisting with instrumental activities of daily living (IADLs)

Home Making

Health conditions such as disabilities, illnesses, injuries or age-related conditions may limit your loved ones’ capabilities. We understand that at times, they can no longer do the things they used to or may experience difficulties in accomplishing their activities of daily living. Through the services we offer, we can guarantee that they are receiving the care they deserve.
